Insurance and Payment information

The Clinic accepts Medicaid, Medicare and several Managed Care Plans (Commercial plans, Medicaid plans and Medicare Advantage) as well as sliding scale fees. Please call to determine insurance eligibility.

It is the client’s responsibility to:

• Provide all insurance information during screening call and bring insurance card to intake appointments, and each session thereafter
• Pay copayment (if applicable) at each appointment
• Pay for sessions until deductible is met (if applicable)
• Provide new insurance information as needed
